Glaucoma treatment

Glaucoma is a chronic, progressive disease that damages the optic nerve. Glaucoma is accompanied by an increase in intraocular pressure, a narrowing of the field of vision, and a decrease in visual acuity.

In the absence of treatment, there is an irreversible decrease in visual acuity, and sometimes complete loss of vision.

Most people become aware of a problem only when there is significant vision loss (when there is damage to more than 50% of the optic nerve). Therefore, it is extremely important to diagnose and treat glaucoma in the initial stages

Types of glaucoma:

– primary open-angle glaucoma (the most common type)

– secondary glaucoma (the result of other diseases, injuries or surgical interventions)

– angle-closure glaucoma, or an acute attack of glaucoma – a sudden significant increase in intraocular pressure, accompanied by unbearable pain in the eye and headache, nausea and sometimes vomiting.

It is an urgent condition that requires an urgent visit to an ophthalmologist.

– congenital glaucoma

Diagnosis of glaucoma:

– Visual acuity check

– Measuring intraocular pressure

– Ophthalmological examination of the anterior segment of the eye and fundus, with a focused examination of the optic nerve

– Gonioscopy – examination of the angle of the anterior chamber using a special high-diopter lens

– Computer perimetry – determination of the field of vision

– OCT of the optic nerve

– Daily tonometry (ICare Home) – measurement of intraocular pressure every hour, including at night, to rule out or confirm an increase in pressure throughout the day.

Treatment of glaucoma

Glaucoma cannot be cured, all treatment methods are aimed at stopping the progression of the disease!

Today, there are the following methods of glaucoma treatment:

  1. Medical treatment – prescribe drops that reduce intraocular pressure. It is possible in the initial stages of the development of glaucoma.
  2. Laser treatment is also used in the initial stages of glaucoma, when drug treatment is not effective enough:
  • Cyclophotocoagulation
  • Laser trabeculoplasty
  • Laser iridectomy
  • Peripheral iridectomy
  • Border photocoagulation (pulse) Laser trabeculoplasty.
  1. Microsurgical treatment – in the absence of intraocular pressure compensation, it is necessary to carry out surgical treatment as soon as possible to preserve vision.
The following methods of surgical treatment of glaucoma are performed in the Ophthalmology Group

–         Cataract phacoemulsification is not a method of glaucoma treatment, but it is the first step to reducing and stabilizing intraocular pressure.

–         Non-penetrating deep sclerectomy is a special new path for the outflow of intraocular fluid created in the scleral membrane of the eye, which leads to a decrease and normalization of intraocular pressure.

–         Implantation of the Ex-Press valve is a modern method of surgical treatment of glaucoma, in which a special drainage device is used. This procedure is the most effective and least traumatic among all other methods of surgical treatment of glaucoma.

–         Combined surgical treatment includes phacoemulsification of cataracts and one type of anti-glaucoma surgery (in cases where the patient is simultaneously diagnosed with cataracts and uncompensated glaucoma).

Transscleral cyclophotocoagulation

is a method of external laser exposure to the structures of the eye that are responsible for the production of intraocular fluid, to reduce intraocular pressure.

TSCFC is a safe and effective method of reducing intraocular pressure in patients with severe uncontrolled glaucoma in cases where neither medical nor surgical treatment is sufficient. Transscleral cyclophotocoagulation allows for the preservation of residual vision and the eye as an organ (in the terminal painful stages of glaucoma).
